Advanced Clinical Practitioner - closes 23 June 2021

The post holder will be an enthusiastic, motivated and experienced Registered General Nurse, Paramedic or Advanced Nurse Practitioner, who can act confidently and competently within their professional boundaries to provide clinical care for patients requiring home visits, in particular a focus on patients living with nursing and residential homes across Dover Town.

Clinical care will include telephone triage, initial history taking, face to face assessment, physical examination, diagnosis, treatment planning, care planning, education support to care staff, case management, referral, evaluation of care and when necessary palliative care support. They will demonstrate safe, clinical decision-making and expert care, including physical assessment and diagnostic skills.

Care Coordinator - closes 30 June 2021

We are looking for an exceptional care coordinator to join our friendly, supportive, and progressive organisation. Dover Town PCN has three practices and our care coordinator will be a key member of the administration team in providing person centered care for our patients.

You will support the clinical team to provide services which put the patients needs at the heart of the care discussion. Your role will enable patients to access healthcare services in a timely manner and support the Network to improve the health of the local population.

You will be working with our partner health and social care organisations, patients, relatives, carers and voluntary organisations to manage the administration of Multi-Disciplinary Team meetings.

You will need to have great interpersonal skills, be a good listener and have experience of working positively with people facing complex social and emotional challenges.

PCN Pharmacy Technician - closes 30 June 2021

The purpose of this role is to lead improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care. The post holder will help patients to get the best from their medicines through effective medicines reconciliation after discharge and on other transfers of care, medication use reviews, identification of patients suitable for electronic repeat dispensing, promotion of online ordering, providing assurance of monitoring of high risk drugs, and handling of prescription queries within the areas of competence.

The role will also include training and development of practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.

Practice Nurse - closes 01 July 2021

The High Street Surgery have an exciting opportunity to join our friendly, innovative, developing and forward thinking team. We are seeking an enthusiastic, committed and caring nurse to enhance our current team of 2 nurses, 2 nurse practitioners and 2 HCAs.

We are looking for a practice nurse to work either full-time or part-time Monday - Friday (excluding Thursday) between 8:00 - 18:30 (30-35 hours approximately).

To provide a high quality care service based on the needs of the Practice population which includes treatment, screening, preventative care and patient education.

Teamwork is at the heart of our nurse team. We work hard but effectively with a passion for improving health outcomes for all our patients. Paramedic Practitioner/Nurse Practitioner - closes 18 July 2021

A great opportunity to come and join a thriving GP practice in Dover.

We are looking for a Paramedic Practitioner/Nurse practitioner to work alongside the GPs seeing our minor illness patients.

To assess, diagnose, treat, refer or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.

The post holder will use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment.

This we be working 24 hours a week over Wednesday - Friday.
